Private Health Insurance Rebates
Private health insurance rebates apply to the services listed above. We offer convenient on-the-spot claiming through HICAPS with most major private health funds, allowing you to claim your rebate immediately at the time of your appointment.
Please note that rebate amounts vary depending on your health fund, level of cover and annual limits. We recommend contacting your private health insurer directly if you would like to confirm your expected rebate prior to your appointment.
Please Note:
- Hydrotherapy Classes: An initial physiotherapy assessment is required prior to commencing hydrotherapy to assess suitability and develop an individualised programme tailored to your specific needs and goals.
- Gym-Based Rehabilitation: An initial physiotherapy assessment is required prior to commencing gym-based rehabilitation to assess suitability and develop an individualised programme tailored to your specific needs and goals.
- Home Visit Physiotherapy: Home visit appointments must be arranged directly with the clinic. Please call (08) 9244 8588 or email chris@lakegwelupphysio.com.au to book. Travel time will need to be factored into appointment scheduling.
- Extended Consultations (60 Minutes): Extended appointments require direct booking with the clinic. Please call (08) 9244 8588 or email chris@lakegwelupphysio.com.au.
Concession Discounts
We offer a 10% discount for eligible:
- Commonwealth Seniors Health Card (CSHC) holders
- Pensioner Concession Card holders
Please present your concession card at the time of your appointment.
Cancellation Policy
To allow sufficient time for other patients requiring treatment:
- A $50 fee may apply for appointments cancelled with less than 12 hours’ notice.
- A $50 fee may apply for non-attendance.

Medicare, DVA, NDIS, Workers' Compensation & Motor Vehicle Accident Claims
Lake Gwelup Physiotherapy accepts referrals and funding through a range of government and third-party schemes, including:
Medicare Chronic Disease Management (CDM) Plans
If your GP has referred you under a Medicare Chronic Disease Management (CDM) Plan (previously known as an EPC Plan), Medicare rebates may be available for eligible physiotherapy appointments. As of January 2026, the Medicare rebate for physiotherapy services provided under a CDM Plan is approximately $61.80 per session. Medicare rebate amounts are indexed periodically and may change over time.
Lake Gwelup Physiotherapy does not bulk bill Medicare physiotherapy appointments. Patients are required to pay the standard consultation fee on the day of treatment, with the applicable Medicare rebate then processed and paid directly into their nominated bank account.
DVA, Workers' Compensation & Motor Vehicle Accident Claims
For approved DVA, Workers' Compensation and Motor Vehicle Accident (ICWA) claims, there are no out-of-pocket expenses for physiotherapy treatment.
NDIS Participants
Lake Gwelup Physiotherapy welcomes self-managed and plan-managed NDIS participants and can assist with a wide range of rehabilitation, mobility, balance and functional goals.
